Older People's Mental Health Service

We can offer you mental health assessments, diagnosis, and on-going care to help with mental health conditions, including:

  • Dementia (we work with younger and older adults with dementia)
  • Depression
  • Psychosis
  • Severe anxiety

We want to help you stay independent and, where possible, avoid having to go into hospital for treatment.

Once you’ve been referred into our service, we’ll make an appointment with you assess your needs and create a personalised care plan to make sure you get the right care.

We can visit you in your home, residential home, our outpatient clinics, community hospitals or in your local GP surgery – just let us know what’s easiest for you.

During your first appointment, you’ll get the chance to ask any questions and discuss your possible treatment options.

We can help you in a number of ways, including:

  • Assessing and diagnosing your condition
  • Providing medical care
  • Referring you to our Talking Therapies service
  • Offering you group and one-on-one therapy
  • Referring you into occupational therapy
  • Providing support and information for your carers
  • Access to inpatient services (for 24 hour care)

Our service is made up of a number of specialist teams, including:

  • Community Mental Health Team
  • Memory Service (Memory Clinics)
  • Home Treatment Team
  • Care Home Support Team
  • Inpatient Services
  • Older People's Mental Health Liaison Team

We generally work with people who are over the age of 75 but, in some cases, we’ll work with younger people too.

To access our service, you can self-refer or ask your healthcare professional to refer you through our CPE team.

If you self-refer, we’ll ask you some question about your level of need and general physical health as this will help us decide which service will work best for you.

You can also be internally transferred or referred to us by other teams in our trust.
