Join HealthMakers

HealthMakers can help you better manage your long term health condition and significantly improve your quality of life through their six week self-management course.

Once you've been on the course, you can help others in similar situations by:  

  • Offering peer support to teach others how to manage their long term health conditions. So far our HealthMakers have made a difference to the lives of 50 people in Bracknell
  • Delivering training to help others become HealthMakers and make a difference
  • Acting as Patient Partners who work closely with local health services to improve patient care and quality of life

The HealthMakers self-management course teaches you how to manage your long term health condition.

Through a series of interactive workshops, you'll cover a range of topics, including:

  • The benefits of good self management and how it can improve your quality of life
  • Anxiety management
  • Goal setting
  • Living with a long term condition
  • Developing a good and effectively relationship with your healthcare professionals

The course, which was was commissioned by the NHS, is run by two fully qualified facilitators.

Who can access the course?

This course is available to anyone living with a long term health condition in East Berkshire (Ascot, Bracknell, Maidenhead, Sandhurst, Slough and Windsor). To get the most from your experience, you need to be willing to work with others to improve your quality of life.

What are the benefits?

 During the course, you'll:

  • Gain knowledge, skills and attitudes for good health and wellbeing
  • Develop listening skills and the ability to learn from others
  • Create and participate in strong local networks and groups around your condition
  • Reduce your need for medication and hospital admissions
  • Be inspired and inspire others..

We’re looking for new HealthMakers to join our team and support others to self-manage their long term health conditions.

You’ll be trained in how to facilitate a self-management course and given all the content and knowledge needed to confidently deliver one, including:

  • Goal setting
  • Anxiety management
  • How to facilitate a course
  • Public speaking
  • Problem solving
  • Other optional modules

Once you’re trained, we’ll set your courses up so it fits with your availability but we do expect you to deliver at least two courses a year.

You’ll be able to claim expenses for your travel and any carer costs. If you’re a healthcare professional, we can cover any back fill costs and provide you with a certificate to evidence continued professional development (CPD).

We’re looking for people who:

  • Have experienced living with a long term health condition
  • Are willing to listen to and consider different view points
  • Can provide constructive and on-going feedback to others
  • Can manage and plan your own time

Any trained HealthMaker can become a Patient Partner and help us influence local healthcare in Bracknell to improve patient care.

We’ll provide you with the training and support you’ll need as well as reimbursing you any travel and carer costs.

Once you’re a Patient Partner, you’ll be given the chance to get involved with one or more of these groups:

  • Patient Partner position on CCG Boards and Committees
  • Condition specific groups which match your own interest and experience
  • GP Practice patient group development

Essential criteria:

  • Have experienced living with or supporting someone with a long term health condition, or having a genuine interest in improving patient care
  • An interest in NHS structure and wider health services
  • Can understand and represent the wide range of opinions which reflect all patients with long term health conditions
  • Can read the provided documents and attend regular meeting (or can be supported to do this)

Other criteria:

  • Can confidently express views and opinions at meetings with healthcare professionals (we will provide support and training to help you with this)
  • Are registered with a GP practice in the Bracknell and Ascot CCG area
  • Are willing to listen to and consider different view points
  • Challenge current thinking in a creative and supporting way
  • Can provide constructive and ongoing feedback to others
  • Can manage and plan your own time
