We can provide support for adults who have been diagnosed with a learning disability.

Once you’re referred into our service, we can support you with advice, an assessment, and a range of treatments and therapies.

Our team is made up of experienced healthcare professionals who will work together to give you a personalised care plan, including:

  • Nurses who can help you get a health check with your GP, look after your medication and find out about behaviour and epilepsy management
  • Health support workers
  • Occupational therapists who can help you improve your independence by carrying out day to day activity and assessing you for specialist equipment
  • Physiotherapists who can help reduce pain and improve mobility issues, including specialist equipment like wheelchairs or mobility aids
  • Psychiatrists who can help you manage your emotions and make life less stressful as well as providing any medication you might need
  • Psychologists who can help you cope with your emotions and improve your communication skills
  • Behaviour specialists
  • Dieticians
  • Speech and language therapists who can improve your communication skills and help with any eating or drinking problems

We also work in partnership with social workers and social service assessment officers to make sure you’re getting the best support possible.

Hospital Passport

We've also produced a Hospital Passport that you and your carers can fill in each time you go into hospital. This will help your doctors and nurses support you in the best way possible.
