Supporting your appointment

Your e-Health Passport is a part of your digital care records which helps us understand you, or the people you care for, better.

If there is anything important to you, such as the way we speak to you, or the way we provide your care, let us know and we can add it to your record so you only need to tell us once. 

This could include your faith, ethnicity, gender identity, pronouns, reasonable adjustments, or any access or neurodiversity needs.

You can share anything that helps make your care more comfortable or personal. For example:

  • How you prefer us to communicate: “I need a British Sign Language interpreter,” or “I need information in clear, simple language.”
  • Any access or sensory needs: “I need step-free access,” or “Busy waiting rooms can feel overwhelming for me.”
  • Faith, cultural or personal preferences: “I prefer to be seen by a clinician of the same gender, where possible,” or “These are the pronouns I use…”
  • Support needs: “I struggle with forms – please help me complete them,” or “I need my carer or family member with me at appointments.”

Sharing this information means that we everything important about you in one place, helping us make your appointments as comfortable and personalised as possible.

The reasonable adjustments flag also links to your National Care Record. This means that if another provider, such as your GP, records any reasonable adjustments, we can see them too – so you won’t need to repeat yourself.

You don’t need to fill anything in. Just ask any member of staff to update your e-Health Passport with what matters to you.

Neuro-inclusion Passport

Alongside the e-Health Passport, you can also use our Neuro-inclusion Passport.

This is a physical form designed to help neurodivergent people communicate their needs. You can complete yourself, or ask a member of staff to support you.

The Neuro-inclusion Passport includes questions about sensory processing, communication preferences, what might make you feel anxious or overwhelmed, and what we can do to support you. 

Once you have completed it, please print it and bring it to your appointment. Hand it to your clinician and we’ll add it to your e-Health Passport so any of our staff working with you can see it.
