Service overview

The Berkshire Integrated Hub is our single point of access (SPoA) for a number of community services and Wokingham Adult Social Care.

We process referrals for adults aged 18 or over to ensure you're directed to the right service for your needs. We do not deliver patient care as part of our service.

Our team is made up of administrative staff and registered nurses and we work in partnership with the WestCall GP Out of Hours service.

Accessing our service

We accept referrals 24/7, including Bank Holidays, although only urgent referrals will be processed between 6pm to 8am.

Referrals to some services must be made by healthcare professionals, such as your GP or a hospital clinician, via email or phone call. Details on how particular services take referrals can be found on their webpage.

We only accept self-referrals for East District Nursing, West District Nursing (for end-of-life care), and initial contact with Wokingham Adult Social Care. All referrals can be made via phone call. Our referral coordinators will ask for your contact details and gather information about your circumstances.

When we receive all the necessary information for a referral, we will direct your referral to the most appropriate service and our registered nurses can offer clinical support.
