Neuro-rehabilitation (community based)
We can help you manage or recover from a neurological condition or diagnosis (such as head injuries, Stroke, Multiple Sclerosis and Parkinson’s disease) through neuro-rehabilitation.
We’ll visit you in your own home or a community location and work with you to make daily life easier. Our aim is to help you improve your independence, get back to work or start enjoying leisure activities again.
Once you’ve been referred into our service, we’ll book an assessment so you can tell us about the difficulties you’re having. We’ll then work with you to develop some goals you can achieve within 12 weeks.
You’ll work closely with our Occupational Therapists, Physiotherapists, Speech and Language Therapists, Psychologists and Therapy Assistants to improve your:
- Mobility and movement
- Ability to carry out everyday care activities
- Depression, distress or anxiety
- Cognitive functions
Depending on how quickly you make progress, we’ll book in one to three appointments a week for up to 12 weeks.
To use our service, you need to be over 18, registered with a GP in West Berkshire and be diagnosed with a neurological condition.
If you’ve not used our service before, you need to be referred to us by your GP.

We’ll do a quick assessment based on the information your GP provides and, if we can provide the right care for you, we'll send you a confirmation letter and call you to book your full assessment.
Please be aware that we currently have a waiting list for our service but will try to see you as soon as possible.
