Intermediate Care

Our personalised care packages can help you get an early supported discharge from hospital, or prevent you from having to go to hospital at all. We do this by helping you improve and maintain your independence.

If it’s appropriate, we can also give you end of life support.

Once you’re referred to our service, one of our clinicians will come and assess your needs. They’ll help you set some goals to help you improve your independence.

You’ll then work closely with our team of registered nurses, occupational therapists, physiotherapists, social workers, intermediate care therapeutic assistants and ‘reablement’ assistants to achieve your goals.

If you need it, we can also provide you with equipment to improve your mobility and help you stay at home longer.

Depending on your needs, we may be able to offer you different treatment support:

  • A two hour rapid response if you’re in the community and at risk of being admitted to an acute hospital. Our support can last up to 14 days or until you’re no longer at risk
  • A two hour rapid response if you’ve just arrived home after being discharged from the CDU or A&E. We can also help if you’ve been given a holding bed at an acute hospital within 48 hours of admission
  • A rehabilitation programme that can last for up to six weeks with clear goals to help you improve your independence
  • A time limited emergency night sitting service to give you extra support if you’re vulnerable or at risk of an unplanned hospital admission
  • End of life/palliative care if you want to remain at home

Our Intermediate Care service is available to anyone over 18 and registered with a GP in Reading, Wokingham or West Berkshire.
