Health Outreach Liaison Team (HOLT)
If you’re homeless, living in a hostel or have difficulties accessing mainstream health services, you can visit our clinics and speak to a member of our Health Outreach Liaison Team (HOLT).
At our clinics, you can meet our registered general nurses, registered mental health nurses and senior support workers.
We’ll carry out an initial assessment and, if possible, provide you with a treatment.
If we can’t help at our clinics, we’ll refer you to the right service or agency that can help. We’ll then liaise with the service or agency to make sure you get the care you need. This might include registering with a local GP, dentist or optician.
You don’t need to be referred or book an appointment to visit one of our clinics. You can drop in at any time during our open hours.
However, you can be referred by a healthcare professional or support services. They do this by filling out a referral form or by calling us directly.

Every Monday, 11am to 1.30pm, at the Churches in Reading Drop In Centre (CIRDIC), 1 Berkeley Avenue, Reading, RG1 6JT.
Every Tuesday, 7am to 9am, in partnership with St Mungo’s, Reading, to offer help to anyone found sleeping rough.
Between Tuesday and Friday, we hold a number of clinics on a referral basis at different locations. These include approved premises, Launchpad and 1st Stage housing hostels.
