Community based Neuro-Rehabilitation (CBNRT)

We can help you manage your neurological condition, such as stroke and Parkinson’s disease, through our neuro-rehabilitation service.

Our Community Based Neurological Rehabilitation Team (CBNRT) service may be able to provide support if you’re coping with the effects of a neurological condition.

Our team is made up of neuro-rehabilitation specialists, who can help if you’re experiencing conditions such as stroke, Multiple Sclerosis and Parkinson’s disease, or a serious head injury.

We can help you manage your condition in the comfort of your own home, or somewhere in your community such as your place of work.

Our aim is to help you to be as independent as possible. During your rehabilitation, we may work with you on things like improving your memory, helping your ability to manage daily tasks, assessing your physical abilities and helping you communicate.

When you’re referred to us, we’ll review your needs and offer the right treatment options for you.

Please be aware that we currently have a waiting list for our service but we’ll try to see you as soon as possible. 

To access our service, you need to be aged 18 or over and registered with a GP in West Berkshire. You'll also need to have been diagnosed with a neurological condition and have defined rehabilitation goals.

Our service is community based and we have teams working from West Berkshire Community Hospital, Cremyll Road, Wokingham Community Hospital and St Mark’s Hospital.
