Make an adult mental health referral

All referrals to our mental health services can be made by contacting our Gateway team.

Your referral will depend on the referral process and criteria for the service.

Self-referral

Some of our services, such as NHS Berkshire Talking Therapies for Anxiety and Depression, support self-referrals, meaning you can get in touch and ask for an appointment.

Please check with the service first to know more. You may need to contact your GP first.

Non-GP referral

You can use our Non-GP referral form if you are not a GP, but you need to make a referral to Berkshire Mental Health Services via The Gateway and you are a:

  • Health or Care professional (excluding GPs), or
  • Supporting care agency, or
  • Third sector

This form is only for patients aged 18 to 75 years old.
