Service overview
We offer personalised therapy for patients on each of our pathways, which includes hospital discharge, community physio, and falls. The therapy sessions we provide for house bound patients will help you regain independence at home and maintain strength and balance.
We do this through a rehabilitation programme to help you improve and keep your independence.
Our team is made up of:
- Occupational Therapists who can help you build your independence to do your daily activities.
- Physiotherapists who can help improve your mobility and physical strength.
- Therapy Assistants who can guide and help you throughout therapy sessions Depending on your needs and referral, we will recommend the appropriate therapy plan.

Accessing our service
You can use our service if you’re aged 18 or over, and registered with a GP in Reading, Wokingham, or West Berkshire.
You can be referred to us by GP or healthcare professional, through our Integrated Hub: integratedhub@berkshire.nhs.uk
Once we have your referral, we'll contact you to talk about next steps and waiting times.
We'll then arrange an assessment in your own home.
Preparing for an assessment
At your appointment in your home we'll talk with you about your personal recovery goals, and create a treatment plan together. You may wish to have a family member or carer present at the assessment.
We suggest working towards short-term, realistic goals such as improving your mobility using appropriate walking aids or developing a strength and balance exercise programme.
We may also complete assessments of your activities of daily living. This may include personal care such as washing, dressing, and meal preparation assessment.
The assessment can take between 1-2 hours and as part of the home assessment we may review and assess your home environment for appropriate equipment to support your needs.
We may guide you to another service if we feel it will help with your rehabilitation.
You may need care calls alongside our therapeutic intervention, and these are usually provided by the Local Authorities.
Managing your health
Following the assessment and depending on your needs, there are steps we can put in place to maintain your safety and independence in your own home, such as:
- Providing and assessing equipment to help you complete daily activities.
- Creating a personal exercise programme to boost your strength, balance, and to support your short-term goals.
- Referring you to other services as required to support your needs.

Do you provide the equipment?
Yes we do, if we feel it's the right fit for the patient's needs.
Do you offer home assessment?
Yes, we offer home assessments. It's the only type of assessment we provide. This allows us to better understand your environment and tailor our recommendations to suit your needs.
Can I still have support if I’m working?
Yes if you're housebound. This service is only for people who need help at home.
I have been provided with equipment that I no longer need, what do I do?
If you have equipment that’s no longer needed, please contact the Equipment Service to arrange a collection. Certain recycling centres will also take old equipment, we recommend contacting them directly to confirm.
How long will I receive this service?
This is a short-term service tailored to your individual needs, and your duration in the service will be discussed with your healthcare provider.

Is there any information for friends, family and carers?
If you’re a family member/carer or you look after someone, there’s practical, financial, and emotional support available to you from a range of local communities and national organisations.
How do I request an interpreter?
We can provide language translation and interpretation support whenever you visit or contact us.
Tell our staff which language you prefer to use, and we will make sure the right communication support is in place for your assessment and treatment. Please let us know as early as possible so we can ensure the appropriate translation or interpreting support is available.

- video remote interpreting (VRI)
- face‑to‑face interpreting
- written translation
- British Sign Language (BSL) interpreting
How can I get information in an accessible format?
Anyone with a disability, impairment, or sensory loss has the right to receive information in a format that meets their needs. Under the Accessible Information Standard (AIS)—a legal requirement for all health and adult social care providers—we must ensure that people who use our services, including carers and families, can understand the information we provide and communicate effectively with us.
We can offer information in a range of accessible formats, including:
- British Sign Language (BSL)
- large print
- braille
- audio
- easy Read
- text message
- face‑to‑face support with a carer or advocate present
If you need information in any of these formats, please tell a member of our team and we will make sure your communication needs are met.
Are service dogs allowed to my appointment?
Yes. You are welcome to bring your registered service animal, such as a guide dog, hearing dog, medical alert dog, or other trained assistance dog to your appointment.
These animals are recognised as essential support and are permitted in most areas of our services.
To help us prepare, please let the team know before your appointment if you will be attending with a service animal. This allows us to make sure the environment is safe and comfortable for you, your animal, and other patients.
Please note that service animals may not be able to enter certain restricted clinical areas for safety or infection‑control reasons, but we will always work with you to find an appropriate alternative.
Treating our team with respect.
Respect is important.
We will be polite and kind and we expect that you treat our staff in the same way.
Abuse, hate and discrimination against our staff is unacceptable.
We will take strong action against anyone who is verbally, racially, physically, or sexually abusive to them.
This includes contacting the police to prosecute, and stopping future access to our healthcare services.
