Overview

We can provide you with a copy of the information we have about you and your care, or the records of someone you have the appropriate legal right of access to, such as their consent or if you have Power of Health and Welfare Attorney.

To do this we encourage you to submit a request form to ensure we provide you with all of the information you are requesting.

We may also ask you to verify your identity by providing us with ID. 

Confidentiality

We take the safety, security and confidentially of our patients and their information very seriously.

We only disclose patient information to those who have a legitimate need to see it (such as other health professionals involved in the patient’s care) or those with a legal right (such as family members who has a patient’s consent to release the information).

We will not provide any medical records until we are assured of your identity, or that the person requesting records on your behalf have a legal right to the information.

In some exceptional cases, we may be required by law to provide medical records to other services, such as the police.

If appropriate, we will notify you of who is requesting this information.

  • Complete a FOI request form

Submit your request

To request records, you need to provide forms of ID so we can verify your identity.

If you wish to request someone else’s records, you will need a signed letter of consent from them, or the appropriate legal document such as Power of Health and Welfare Attorney.

You can find full list of the ID and consent you need to provide in the request form.

Complete the request form, and send this to the relevant department or team. 

Please include your full name and address, your contact details, and a clear description of the information you require.
