Rapid Response Service
If you’ve had an illness or injury that’s made it harder for you to look after yourself, we will support you in a crisis. We will help you remain in your home and avoid being admitted to hospital unnecessarily.
Our aim is to prevent you having an unplanned hospital admission.
Once you’ve been referred into our service, we will provide you with extra support in your home while you recover. This reduces the chances of you needing to use hospital based services again.
If you’re at risk of being admitted to an acute hospital, we will visit you within two hours and assess your needs.
Our team of occupational therapists, physiotherapists and community nurses will decide on the best course of action. They can help you with personal care and practical tasks, such as shopping and housework.
We can also provide end of life support if you choose to die at home.
Once referred, we’ll provide you with a rapid response service within two hours if:
- You’re in the community and at risk of being admitted to an acute hospital
- You’ve been discharged from a hospital Clinical Decision Unit (CDU) or Accident & Emergency (A&E) department
- You’ve been given a holding bed within an acute hospital within 48 hours of admission

You can be referred into the service by your GP or healthcare profession through our Health Hub. The hub manages all referrals for Berkshire Healthcare community services.
