Rapid Assessment Community Clinic

If you’ve been diagnosed with a long term condition or are frail, we can make sure that you get access to treatment as quickly as possible to avoid complications and unnecessary visits to hospital.  

Once you’re referred into our service, we’ll contact you to book an appointment. If you’re referral has been made before 4.30pm, we can see you the same day to carry out an assessment.

We’re based at St Mark’s Hospital in Maidenhead but, if you’re not able to come see us, we can visit you at home or in the community and do your assessment there.

Once we’ve completed your assessment, we’ll provide you with a personalised care plan to make sure you’re getting the right healthcare, some self-care information and any relevant patient and carer information leaflets.

Our team – made up of nurses, specialists, consultants, physiotherapists and occupational therapists - will work closely with your local community healthcare services, social services and local charities to develop the best support package for you.

To access our service, you’ll need to be referred by your GP, the ambulance service or any other healthcare professional.

Unfortunately, we’re not able to accept self-referrals.

If your referral is made before 4.30pm, we may be able to see you for an assessment on the same day.
