Older People's Mental Health Liaison Team
If you’re 65 or older, we can provide you with support for emotional, psychological or memory difficulties during your stay at Royal Berkshire Hospital or Wexham Park Hospital.
We’ll work with the staff on your ward to make sure that you’re receiving the right care and to help speed up your recovery.
We can help you if you’re struggling with:
- Dementia and/or delirium
- A new mental health difficulty or existing mental health problem which needs support during your stay in general hospital
- Adjusting to a physical illness
We can also help you if you’re under the age of 65 and dementia is suspected.
Once you’ve been referred to our service, we’ll come and visit you on your ward. Depending on the urgency, you could be seen within two days of your referral.
We’ll assess your needs and collect some information from you, your relatives and your healthcare professionals. We’ll always ask for your permission first before we contact anyone.
Once our assessment is finished, we’ll discuss your treatment options with you and your family or carers. We’ll then put together an individual care plan to make sure you get the right support.
To access our service, you need to be referred by a member of staff at the Royal Berkshire Hospital or Wexham Park Hospital.
Staff will refer you if they think it can improve your wellbeing during your stay in hospital.
