Slough Mental Health Care Pathways

If you’re experiencing a severe mental health condition, such as schizophrenia or bipolar disorder, we can help assess your needs and guide you through a treatment and recovery plan.

We can help you if you’re registered with a GP in Slough, and experiencing a severe mental health condition. It could be that you’re having delusions and hallucinations (psychotic condition), or a depressive disorder or anxiety (nonpsychotic condition).

We can also offer support if you have a personality disorder, or relying on substances such as drugs and alcohol to help cope with your mental health issue (dual diagnosis).

When you’re referred to us, we’ll offer a full assessment to better understand your situation. As well as looking at your immediate concerns, we’ll talk to you about your life growing up, treatment history, and your personal goals and aspirations.

From your assessment, we’ll work together with you to create an agreed, personal care plan which builds your confidence and independence to manage your condition. Treatment type and the length of care will depend on your needs, and your progress.

To help you with your recovery, we can offer guidance on a number of things such as:

  • Advice on socialising in areas like employment and education, daily activities like visiting a bank, and personal relationships
  • Interventions on types of medication e.g. anti-psychotic and anti-depressant medication, to help you manage
  • Access to therapeutic interventions to help you talk about your thoughts, stress you’re having and any emotional challenges
  • Guidance for your family and carers about your condition, and how they can best support you
  • Support on suicide prevention if you’re feeling vulnerable

Assessment and care from our team can be provided either at our community mental health centres across Slough, or in your home if we’re able to.

To access our service, you must be diagnosed with either a psychotic schizophrenia or bipolar, or nonpsychotic condition. You must also be registered with a GP in Slough.

If after your assessment we think you’d benefit from support from one of our other specialist services, we’ll discuss the options with you and make a referral.

Unfortunately, we don’t accept self referrals.

If you’re experiencing a severe mental health incident, you can get help by calling 111, booking an emergency GP appointment or visiting A&E.

If you feel you’re at risk of harming yourself or others, please call 999 straight away.

Our team is based at New Horizons in Slough, but our courses and workshops are provided right across Slough. Please contact us to arrange a visit to us.

We're available for contact 9am to 5pm, Monday to Friday.

If you need help outside of these times, you can leave a message with our team and they’ll contact you the following day.

Services will be available through our Slough Community Mental Health Team (CMHT), in your home, your local GP surgeries and other local venues.

Friends in need

If you’re coping with some mild forms of depression or anxiety, you can find support from Friends in Need. They’re a peer support group, by the Mental Health Charity, Mind.
