Earlier Supported Discharge (Stroke)

This multi-disciplinary service provides Earlier Supported Discharge to acute stroke patients, stroke-specialised rehabilitation to patients in their homes within one working day upon discharge from the stroke unit for up to 6 weeks.  The disciplines within the team are specialist Physiotherapists, Nurses, Occupational Therapists, Speech and Language Therapists, Clinical Neuro Psychologists and Therapy Assistants.

It provides intensive stroke specialist rehabilitation to patients in the community and stroke specialised care, support and information to patients and their carers.

Referrals are made through discussion at the twice weekly MDT on the Acute Stroke Unit at the acute hospitals in the area and via telephone discussion at other times, with completion of a discharge checklist.  This will be completed by the ESD Stroke Specialist Rehab Nurse or the duty ESD coordinator.

The service is available 08:00 - 16:30 Mon - Fri

Criteria:

• Over 18 and registered with a Berkshire GP.
• Admitted to an acute hospital
• Have a confirmed stroke diagnosis
• Declared medically stable for discharge
