Sustainability and Transformation Plans
The NHS and local councils have come together in 44 areas of the country to develop proposals that will support improvements to health and care.
This is being done as part of the Five Year Forward View, the new vision for how the NHS will operate both now and, just as importantly, in the future. The aim is to help communities be the healthiest they can be, provide excellent patient care and make sure that services are affordable for years to come.
These proposals, called sustainability and transformation plans (STPs), are created across geographic locations and built around the needs of local people.
We're working alongside our neighbouring NHS organisations, local councils and community partners to make services for people’s physical and mental wellbeing more accessible, closer to home and delivered in a more straightforward way.
We’re supporting two STPs - Frimley Health and Care, and Berkshire, Oxfordshire and Buckinghamshire (BOB) Health and Care - and these plans are building on the work which is already underway to achieve changes that local people and local clinicians have told us they want.
