Children's Community Nursing
We provide nursing services for children and young people with acute and complex health nursing needs across Berkshire.
This is made up of Specialist School Nursing and Respite care for children with a nursing need and learning difficulty in Berkshire East. There are some differences in the services offered in the east and west of the county.
The Children's Community Nursing services in Berkshire West is based at the University of Reading, London Road, RG1 5EX. This is an exciting opportunity for the Children's Community Nursing team to work in very close collaboration with the specialist school nursing team, which will ensure that good quality nursing care is delivered in the most efficient way to children and young people across a variety of community settings.
The Children's Community Nursing team continue to provide home visits as required/requested. The nurses are all wearing appropriate PPE at every visit. The level of PPE worn will depend on the clinical task being undertaken.
All nurses have received both Covid vaccines and each CCN performs a Lateral Flow Test twice weekly.

Children’s Community Nursing Berkshire West
Our Children’s Community Nursing service covers Wokingham, Reading and West Berkshire.
We provide nursing services to children and young people up to age 18 with acute and complex nursing needs. We provide nursing services within the child or young person's home or within the community.
We also offer specialist training in helping to care for your child's specific need for parents and carers within their own home, in special schools along side the Specialist School Nurse, and in designated Local Authority Respite Centres.
The team works very closely with the Specialist Schools Nursing team who provide health and nursing care to children attending the special schools below.

Children’s Community Nursing Berkshire East
Our Children's Community Nursing service covers Bracknell, Slough, Windsor, Ascot and Maidenhead.
We provide nursing services to children and young people up to age 18 with complex health needs and learning difficulties. The nursing care can be provided within the child or young person's home.
We also offer specialist training for parents/carers in helping to care for the child's specific need within their own home, in special schools, and in designated Local Authority short break settings.
Each of our Children's Community Nurses has responsibility for and provides heath and nursing care to children attending these special schools:
- Arbour Vale School, Slough
- Manor Green School, Maidenhead
- Kennel Lane School, Bracknell
- Priory School, Burnham, Slough
We provide respite care at our Manor Green Children's Respite Centre in Maidenhead for children and young people with a nursing need who have a learning difficulty.
CCN East - If you need to contact someone, the team can be contact on the usual office number 01753 635017 or your named nurse’s mobile.
If you are a parent or carer and you feel that your child needs a referral to our Children's Community Nursing team, please visit your GP. We accept referrals from health service providers, education professionals and social care workers as long as the child or young person:
- is under the care of a consultant and;
- has a nursing need and;
- is registered with a Berkshire General Practitioner.
If this criteria is met, please make a referral using the Children's Community Nursing referral form.
The Specialist School Nursing service provides care and support to children and young people up to the age of 19 who have complex health needs and are enrolled in the following special schools:
- Brookfields Special School in Tilehurst, Reading
- Avenue Special School in Reading
- Castle Special School in Newbury
- Addington Special School in Wokingham
The Specialist School Nursing Team consists both of Specialist School Nurses and Specialist Nursery Nurses and is based within the four schools.
The team offers an open access service to all children, parents and carers and teaching staff to advise on health, development and medical conditions. They also work in collaboration with the school, the Children and Young People’s Integrated Therapy Team and other professionals to promote a holistic view of your child’s health and wellbeing.
For children that need a specialised nursing healthcare plan, the Specialist School Nurse works with parents and carers to develop a plan covering your child's needs. When the care plan has been agreed with the parent or carer, the nurse will make sure that school staff are trained, so that your child can attend school safely.
The Specialist Nursery Nurses are an integral part of the team and provide individual care to children who meet the Children’s Continuing Care criteria, and need one to one support so that their complex health needs can be met safely during the school day.

Accessing Our Service
Our service offers open access to all children, parents and carers and school staff enrolled or working in the four special schools.
Other professionals working with the children and parents and carers attending one of the special schools can refer to the Specialist School Nursing Team by completing an online referral form on our referrals page.
Once a referral has been received and accepted, the relevant Specialist School Nurse will contact you within seven working days.
