Accident and Emergency Liaison Wexham Park Slough

Type: crisis resolution community service
In partnership with: consultant psychiatrists; staff grade psychiatrists; duty psychiatrists; community mental health professionals; crisis and home treatment staff; Berkshire East GPs.

Description: Our team is comprised of experienced qualified nurses who assess and redirect people attending Wexham Park Hospital A&E Department with mental health problems. We carry out mental health assessments and provide advice to healthcare professionals from the acute general hospital wards.

Common conditions: services are for people with severe and / or enduring conditions: anxiety; bipolar disorder (manic depression); childhood attention deficit hyperactivity disorder (ADHD); childhood autism spectrum disorder; dementia; depression; eating disorders; learning disabilities; obsessive compulsive disorder (OCD); panic; personality disorders; phobias; post traumatic stress disorder; schizophrenia; serious self harm; sexual mental health conditions; substance misuse.

Crisis Response Team - Out of Hours Crisis Service

Type: crisis resolution; community service
In partnership with: local community mental health access (duty) teams; home treatment teams.

Description: Our crisis response team consists of nurses, social workers and support workers to provide ‘out of hours’ support (overnight, weekends and bank holidays) to individuals experiencing mental health crises who need urgent support or treatment. It’s designed to prevent unnecessary hospital admissions by helping people over the telephone, or through out-of-hours home visits.

Common conditions: services are for people with a severe and/or enduring conditions: anxiety; bipolar disorder (manic depression); childhood attention deficit hyperactivity disorder (ADD); childhood autism spectrum disorder; dementia; depression; eating disorders; learning disabilities if combined with mental health need; obsessive compulsive disorder (OCD); panic; personality disorders; phobias; post traumatic stress disorder; schizophrenia; serious self harm; sexual mental health conditions; substance misuse.

You can access our services through: GPs; other community agencies; individuals and carers can make contact with the Crisis Response Team for urgent assessment and support.

Charles Ward

IAPT Increasing Access to Psychological Therapies Service

Type: commissioned by East Berkshire Primary Care Trust
Age group served: adults
Service cover: East Berkshire
Location: Our main office is in Bracknell, but we have satellite sites and clinical space across the East of Berkshire

Description: Offers help for common mental health problems - mild to moderate and moderate to severe. After assessment the most appropriate way forward will be decided, which may include, structured individual work, well-being courses, telephone follow-up, computerised cognitive behavioural therapy, guided self help, short term counselling, links to employment and advice about other services that could help.

Common conditions treated: anxiety, depression, stress and phobias

Melissa Memory Clinic

Type: outpatients; community service
In partnership with: social services; Alzheimer’s Disease Society; Age Concern; primary and secondary care and any other statutory and non-statutory organisations involved in caring for older people.

Service cover: East Berkshire
Location: Slough

Description: assessment and treatment of memory at early stage to moderate stages of dementia. Offers support so that carers are able to manage and care for their relatives at home. Although we prefer to see service users in the clinic, we can offer a flexible service including home assessment..
