Community Mental Health Team (CMHT)

Type: assessment, care planning and risk assessment; psychological therapy and treatment of mental illness; community service

Description: the team provides a secondary care service to adults in the Wokingam Borough Council district aged 18 to 70 years. CMHT brings together healthcare professionals from a variety of disciplines.

Two consultant psychiatrists are integrated within the team and carry specialist leads for Acute Care Team and Home Treatment Team as well as providing a service to generic CMHT service users. The third consultant for Wokingham is based at Prospect Park Hospital with an inpatient consultant. The inpatient consultant may provide initial outpatients’ follow up. The CMHT has the early intervention in psychosis service and a forensic lead within the service structure.

Common conditions treated: services are for people with severe and / or enduring conditions: bipolar disorder (manic depression); anxiety, depression; eating disorders; obsessive compulsive disorder (OCD); panic; personality disorders;
phobias; post traumatic stress disorder; schizophrenia; serious self harm; substance misuse with dual diagnosis.

Child and Adolescent Mental Health Services (CAMHS)

CAMHS stands for 'Child and Adolescent Mental Health Services'. Berkshire CAMHS aims to promote good mental health, and to assess and treat mental health problems in children and young people up to the age of 18 years. We work in six local clinics as well as the Berkshire Adolescent Unit in Wokingham and other community settings.

Community Older Adults Mental Health Service (COAMHS) also known as Community Mental Health Team for Older Adults

Type: community mental health teams; community service
Age group served: older adults
Service cover: Wokingham Borough
Location: Wokingham

Description: specialist team funded by both Berkshire Healthcare Foundation Trust and Social Services integrating both health and social care professionals. We provide a community based service to meet the needs of older people in the Wokingham Borough. Referrals are mainly from GPs and social services.

Our objectives are to:

  • provide early detection, diagnosis and treatment of dementia and other mental health conditions
  • advise on the management of mental health problems
  • promote good mental health
  • encourage and support older people to live safely in their own home as long as possible
  • support carers.

We offer:

  • memory clinics – to assist in the diagnosis and assessment of dementia, including treatment with medication if appropriate
  • outpatient clinics
  • day hospital – for time limited specialist assessment and treatment, providing therapeutic intervention including speech & language therapy, occupational therapy, physiotherapy & psychology
  • carers’ support groups
  • carers’ education course
  • keeping well course
  • social work – to provide advice and information to meet social care needs eg. home care packages, short breaks, financial and benefit advice, care management
  • community psychiatric services – including community psychiatric nurses (CPNs), who are nurses based in the community who visit patients in their own homes.

They can advise on and monitor the effects of medication, give advice on Alzheimer’s disease and other dementias and mental health conditions, and provide support to carers.
